Name and role, and the authority under which you are asking.
Mandate and place
The matter in a line or two, and the jurisdiction it sits in.
Timing and consequence
The deadline you are working to, and what follows from missing it.
Engagement boundaries
Some requests are declined.
Requests that are unlawful, theatrical, or better served by a commodity vendor are declined. The practice stays selective.
